ACT assists people living with HIV/AIDS in meeting emergency expenses through the Client Assistance Fund (CAF), and with obtaining and maintaining housing through the Housing Assistance Fund (HAF).
The application procedures and forms provided below are for case managers. All applicants MUST apply for assistance through a case manager. If you don't have a case manager refer to this list of agencies that provide HIV/AIDS case management, or this list of agencies providing services in the Greater Hartford TGA.

The Client Assistance Fund is a set of services funded by the federal Ryan White HIV/AIDS Program for people living with HIV/AIDS. For Ryan White Part A, applicants must reside in Hartford, Tolland, or Middlesex county. View a list of towns covered by RW Part A Transitional Grant Area (TGA). For RW Part B, preference will be given to those applicants residing in Tolland County or outside of the Hartford TGA. Clients who have capped out of RW Part A funds will also be considered.

ACT has three Housing Assistance Fund programs. Ryan White Part A for the Hartford TGA (Hartford, Middlesex and Tolland Counties), Hartford HOPWA (Hartford County only), and Ryan White Part B (Balance of State – Tolland County and outside of the Hartford TGA). Applicants do not have to apply to a specific program, as that will be determined by the ACT HAF Team.

If you do not have a case manager refer to this list of agencies that provide HIV/AIDS case management. The case manager is responsible for determining your eligibility and must submit a completed application packet to ACT.
