ACT's Care Services department (formerly known as AIDS Connecticut) builds sustainable, accessible, and high-quality systems and services dedicated to eliminating health disparities and improving the quality of life for disadvantaged populations. Our range of programs include direct support for clients as well as assistance for other community and non-profit organizations. Our goal is to increase Connecticut's capacity to ensure that all people in Connecticut have access to the health, housing, and support services necessary to thrive.
ACT offers the following programs to support individuals and social service providers in Connecticut.
Support services including Medical Case Management, Housing Support, and Project TLC.
A safe and supportive haven for individuals living with HIV in the Greater Hartford area.
Assistance for people living with HIV in obtaining and maintaining housing and meeting emergency expenses.
An evidence-based approach to end homelessness as quickly as possible for families and individuals.
A centralized waitlist of all HOPWA funded housing agencies for people living with HIV in Hartford County.
A training program for providers in HIV services, domestic violence, shelters, and transitional living.
Technical assistance for providers in conjunction with our annual Quality Assurance Review Process (QARP).
Integrating data collection systems to better track and improve health outcomes for people living with HIV.
