HOPWA Waitlist - Greater Hartford & Central CT

All Housing Opportunities for People with HIV/AIDS providers are working together, and use a shared waitlist. For more information about this program please contact Megan Auretta, Housing Program Coordinator, at 860-247-2437 x324.

Filling out an application does not automatically add a client to the list. You will be notified when your client is added. Waitlist processes are based on the current Consolidated Plan (Con-Plan) developed by the City of Hartford’s HOPWA Office and is approved by the Department of Housing and Urban Development (HUD). When a client is matched to a program, the client and the case manager (if known) will be contacted by the corresponding program.

Clients on the waitlist are prioritized according to the current Consolidated Plan for the Greater Hartford Region. View current prioritization.

Keep your client’s contact information up to date! If the client’s phone number changes, it is your responsibility to contact ACT to provide updated contact information. If we cannot reach the referred client within sixty days of your selection, we may need to remove the client from the list.

The following are required for ALL Applications:

  1. HOPWA Universal Referral form
    1. Photo identification
    2. Birth Certificate
    3. Social Security card
    4. Verification of HIV Positivity and most recent laboratory results (within last 12 months)
  2. Coordinated Access Network Release of Information
  3. HIV and AIDS Housing Release of Information

Without having these releases and paperwork complete, we cannot add a client onto the waitlist. Optional information includes: Homelessness Verification (completed by a PATH or approved outreach worker or shelter)** and/or Narrative Summary of client situation.

Application Process

Fax all completed applications to 860-761-6711 (Attention: Megan Auretta) or send an encrypted email to HOPWACAN@act-ct.org. Self-referrals can contact Megan at 860-247-2437 x324 for an appointment.

All applications will be reviewed within 5 business days.

  • If all materials are completed, the client will be added to the waitlist and the case manager will be notified.
  • If there is missing information, the case manager will be contacted. If the missing information is not received within 5 business days, the case manager will be contacted a second time. All incomplete applications will be placed in pending status for Thirty (30) calendar days from the initial application. After 30 days, client application will be destroyed. Client may reapply at any time.

The HOPWA Matching meeting occurs once per month. Clients who are matched will be contacted directly by the corresponding agency.

HOPWA Waitlist Matching Meeting

We encourage all agencies included in this project to complete the HOPWA CAN Agency Template form. The purpose of this form is to increase communication between HOPWA committee members/agencies and help streamline the HOPWA Waitlist matching process. Please submit the form to HOPWACAN@act-ct.org prior to the HOPWA Waitlist Matching Meeting.

There are different programs a client may be matched with:

Short Term Residential Program - Residential Program for adults who live in single rooms for up to 90 days while they identify and face complex issues that caused them to become homeless. Working closely with case managers, individuals set and meet goals that will return them to self-sufficiency and independence. (Mercy Housing)

Rapid Rehousing (RRH) – The state of Connecticut Rapid Re-Housing Program (CT RRP) assists literally homeless households living in emergency shelters, on the streets, and in places not meant for human habitation. The purpose of the program is to move households as quickly as possible into permanent housing and achieve housing stability. A combination of housing relocation and stabilization services combined with financial assistance are available. (Healthy Homes, Chrysalis Center (St. Philip’s House)). **Homeless Verification will be required 24 hours before the client is housed. Contact Journey Home for outreach resources.

Permanent Supportive Housing (PSH) - Supportive Housing combines affordable housing, most often through a rental subsidy, with intensive yet flexible support services. These services focus on housing based case management, or assisting the tenant reintegrate into the community by teaching him/her the basic skills of tenancy (Mercy Housing, Hands on Hartford, Chrysalis Center, HRA of New Britain).

Please Note: This is a separate housing waitlist for people with HIV/AIDS and this information is not shared with the local Coordinated Access Network (CAN). If your client is literally homeless and seeking emergency shelter, client must call 211, and dial 3, then 1, to speak with a housing specialist.
